Background

We provide direct and or telephone support in times of mental health crisis outwith normal working hours.

  • Reduce the likelihood of hospital admission by providing immediate practical and emotional support to people in their own homes.
  • Co-ordinate our support with other agencies involved.
  • Support family and carers to maintain their roles during a crisis period.
  • Encourage the development of self help and self management skills.
  • Signpost and provide support to engage with other relevant agencies when required
Information for Referrers

LAMH Source – Out of Hours Crisis Support Service is available to people aged 16-65 living in the rural Clydesdale, Larkhall and Stonehouse area who are experiencing a period of crisis related to their mental health.

This can be direct and/or telephone support.

The service is unable to support people who have a primary diagnosis of any form of dementia, substance misuse or learning difficulties.

How to Refer

A referral to the service can be made in writing using the organisations referral form, or a referral can be taken over the telephone.

  • The person has to be in agreement with the referral taking place and the sharing of their personal information.
  • The referrer must notify of any known risk issues and share any necessary risk assessment and planning documentation.
  • The referrer should outline the reason for the referral and the expected outcome for the person.
  • The referral must also identify who the service should liaise with regarding the support provided.
Communication

We will liaise with the referrer and others engaged in the person’s support during any involvement with the crisis service. Following a period of non contact for 6 months we will contact the referrer/care manager to see if the service is still required and confirm the details held by the service have not changed.

Information for People Referred to the Service

Lanarkshire Association for Mental Health is a long established local charity which provides community based support services for people experiencing a wide range of mental health difficulties. The service provides practical and emotional support by known members of staff at a time when you are finding it difficult to manage because of your mental health. You can be referred into the service by your Psychiatrist, CPN, Social Worker, OT, Hospital Staff or GP.

What to Expect
  • Immediate access to practical and emotional support following receipt of referral.
  • Opportunity to meet staff and discuss how the service could best support you.
  • Direct support and telephone support available throughout a crisis period for up to six weeks.
  • Following the provision of support, a review of what helped and information on further supports available.
  • Once referred you can then access the service when you require our support.
  • Support available to carers, family and friends.
  • If you have not contacted the service for a 6 month period, we will contact your referrer to decide on the next steps.
